2025 年车厂营业代表卡 APPLICATION FOR CUSTOMER SERVICE AGENT CARD 2025

2025年车厂营业代表卡
谨此通知本会于2024年的车厂营业代表卡将于20241231到期。凡从事拖车行业的会员而又无拖欠本会的任何账项者,由即日起可向本会提出申请更新已到期的车厂营业代表卡或申请新员工的车厂营业代表卡。2024年度的车厂营业代表卡可继续使用至31.01.2025

所有有意申请2025年度的车厂营业代表卡者,请遵照下列步骤向本会提出申请:

1)  填妥随函附上的2份表格。如果申请的雇员超过3人,请复印足够的第二份表格即可。第一份表格是填写车厂的资料,只需在第一次申请2025年的车厂营业代表卡时填一次就够了,接下来每次欲替新增加的员工申请时,只需填写第二份表格则可。

2)  每一位申请者请提供下列文件(敬请注意:文件不齐备者,恕不处理 ):           
2.1  身份证复印本
。请把身份证(前后面)和驾驶执照(前后面)复印在同一面。
2.2  有效的驾驶执照复印本。请把身份证(前后面)和驾照(前后面)复印在同一面。
2.3  彩色近照二张(跟身份证的照片同样大小)-(请参阅附图)
2.4a  普险协会会员证复印本(只需在2025年第一次申请车厂营业代表卡时附上)。
2.4b  如车厂非属普险协会会员者,其车厂必须拥有自己的吊车。而所使用的吊车必须是注册在其车厂的名下(请复印车辆注册证以资证明)。如属吊车则车卡上必须注明是拖车或载车的用途。

请特别注意:非属普险协会会员只能申请吊车的营业代表卡。非驾驶吊车的员工一概不准申请车厂营业代表卡。
 

2.5  一份经已证实正确的车辆注册证复印本如申请者属车厂董事(只限一人)请附上公司注册证副本。不必附上车辆注册证复印本。                         
2.6  除董事外,其他申请者(包括经理)必须提呈一张申请者站在其执行任务时所使用的车辆旁的彩色照(请参阅附图)。
                     
3 申请车厂营业代表卡的收费如下:
3.1  如申请者是会员车厂的员工(请参阅
3.3),每张新卡的手续费是RM40.00。如持有2024年的车厂营业代表卡者,申请时请附上2024年的旧卡副本则每张折扣 RM10.00只收RM30.00.(但需附上缴纳公积金和社会保险的表格副本以资证明)

3.2  如申请者不是会员车厂的员工(请参阅3.3的说明),每新张卡的手续费是RM50.00 如持有2024年的车厂营业代表卡者,申请时请附上2024年的旧卡副本则每张折扣RM10.00只收RM40.00.
3.3  为了方便辨认:会员车厂的员工是指那些员工的名字有包括在车厂的薪金表中,并享有缴纳公积金和社会保险制度者。

3.4  如欲本会以快递方式把已完成的车厂营业代表卡寄回者,请每次多付RM10.00的快递费用。

4)  申请2025年车厂营业代表卡的车厂,请严厉遵守下列各事项:
4.1  凡属更新车厂营业代表卡者,必须在领取新卡时把2024年的旧卡退还,如营业代表卡已遗失,请提供 一份遗失车厂营业代表卡的宣誓书,警局报案书及缴付每一张卡的罚款如下:
4.1.1  第一次遗失营业代表卡的罚款是每张卡RM20.00
4.1.2  第二次遗失营业代表卡的罚款是每张卡RM40.00                                            
4.2  如员工已辞职,车厂有责任以最短的时间内通知本会。并负责收回已辞职员工的车厂营业代表卡,连同其辞职信一起交回本会存档。

4.3  如果 辞职后的员工能得到前雇主的函件证明他是清白的并符合4.2的条件,又或者是他能够提供证据证明他是被前雇主拖欠工资或不公平对待等理由,则另一间车厂可马上聘用他。不然他必须经过3个月后才可以被另一间车厂聘用。
4.4  车厂营业代表和吊车员在执行服务时必须:
4.4.1  拥有由总会发出的车厂营业代表卡
4.4.2  身穿总会发行的反光衣
4.4.3  吊车车身必须印有会员车厂注册吊车的车厂名称。车厂营业代表的车辆其车身如是黄色/黑色,则车身两旁由前至后应漆上黑色/黄色条文(请参阅附图)。如果没有漆上黑色/黄色条文,则必须要贴上本会的黄色车贴纸。而执行任务的员工一定要拥有由总会通过本会发出的车厂营业代表卡。
4.4.4  无论是吊车或营业代表的车辆,都必须在车前的挡风镜上方,贴上本会的黄色车贴纸。营业代表车辆的车身如果没有漆上条文但是有贴上本会的黄色车贴纸就算合法。而执行任务的员工一定要拥有由总会通过本会发出的车厂营业代表卡。简单来说,车厂营业代表车辆的车身可免漆上条文 ,但是本会的车贴纸则一定要。同时执行任务的营业代表则必须拥有由总会通过本会发出的车厂营业代表 卡。
4.4.5  持卡者执行任务时,驾驶的车辆如不属其本人所有,则车主必须签署一份同意书让持卡者使用车辆,此同意书必须由车厂盖章签字(请参阅样本)         
4.4.6  穿着正当的鞋子(把脚包住的鞋子),不可穿拖鞋。
4.4.7  赶往发生意外现场时,营业代表车不可使用闪光灯。到了现场,车停下来后才可用。

4.5  如持卡者欲更换服务时所使用的车辆,需把旧卡退还,并重新提供照片一张及所欲更换的车辆注册证副本,本会将免费补发一张给他。

4.6  车厂营业代表在进行服务时,不可使用任何款式的,货车(罗里),客货车,旅休车和四驱车。
凡欲申请2025年车厂营业代表卡者,必须填妥随函附带的表格连同上述文件及所需费用尽快寄到本会秘书处。
 
请给本会两个星期的时间处理申请事宜。申请书没附带费用者,恕不处理 。切记!。 谢谢!

CUSTOMER SERVICE AGENT CARD (CSAC)  2024
Please be advised that the Customer Service Agent Card issued by PAWOA in 2024 will be expiring on 31/12/2024. Those workshops which are involving the towing business & free from PAWOA’s debt are advised to renew their CSAC OR apply new card for their new employees with immediate effect. 
The 2024 CSAC are permitted to use until 31/01/2025.
Those who wish to apply for the CSAC 2025, please follow the following steps:

1)  Please fill up the 2 pages of application forms attached. If you have more than 3 applicants, please make enough copies of page 2 for your own use. Page 1 is for the workshop information, only need to submit once during the 2025 application. 
2)  Every applicant must attach the following documents (documents incomplete will not be entertained):
2.1  Photocopy of My Card (Both sides on the same page )
2.2  Photocopy of Driving License (Both Sides on the same page)
2.3  2 pcs of IC sized colour photograph of applicantSee sample attached)
2.4a  A photo copy of PIAM workshop membership Certificateonly need to submit once during the 2025 application)
2.4b  If the workshop is not a PIAM member, the workshop must process its own registered tow truck (A photocopy of the vehicle registration card is required to prove the ownership).  
For a NON-PIAM member workshop other than the tow truck driver, the rest of the employee are not eligible to apply for a Customer Service Agent Card.
 
2.5  A certified copy of vehicle Registration Card.  
       
For owner/director of a workshop (limited to 1 person), please attach a photocopy of your business registration document, vehicle registration card copy is not required.
2.6  A photograph of applicant standing beside his vehicle is needed (See sample attached)director is exempted.
                 
3)  Application Fees for Customer Service Agent Card are as follow :
3.1  The processing fees for each card for each new applicant who is bona fide employee
of the member workshop(Refer to 3.3) will be RM40.00. For renewal case, please attach a photo copy of 2024 CSAC for a RM10.00 discount, pay RM30.00 only.
3.2  The processing fees for each new card for each applicant who is NOT a bona fide employee of the member workshop (Refer to 3.3) will be  RM50.00.  For renewal case,
please attach a photo copy of 2024 CSAC for a RM10.00 discount , pay RM40.00 only.

3.3  For the purpose of definition, “bona fide employee” means an individual person who is under the payroll of the member workshop, and who is provided with the EPF and SOCSO benefits.
3.4  If you need courier service for the completed cards , please pay extra RM10.00 for the Courier fees.


4)  Workshop Applying for the 2025 CSAC must comply with the following rules:-
4.1  For CSAC renewal case, the workshop should return the 2024 CSAC for an exchange of the new card.  If the CSAC is lost, please provide a police report & a statutory declaration regarding the lost CSAC and pay the following penalty:

4.1.1  The penalty for 1st time lost of CSAC is RM20.00.
4.1.2  The penalty for 2nd  time lost of CSAC is RM40.00.
4.2  Workshop has the responsibility to notify PAWOA as soon as an employee who process CSAC resigned.  The W/shop should get back the CSAC from the  resigned employee & send together with its resignation letter to PAWOA immediately.  

4.3  Any resigned CSAC holder must go through a grace period of three (3) months before he can seek employment in another workshop, unless he can get a letter from his former w/shop certifying that his resignation has complied with 4.2 and  is resigned free from debt and is released to seek other opportunity OR he can provide evident to prove that he was ill-treated or was not paid salary by his former workshop.
4.4  When the Customer Service Agent and tow truck driver are on duty they must :

4.4.1  Process a Customer Service Agent card issued by FAWOAM.
4.4.2  Wearing a reflective vest produced by FAWOAM.
4.4.3  Tow Truck must have its own workshop name printed on the truck. On both sides of the Customers Service Agent Car must lined with a 6 inches high amber strip longitude from front fender to rear quarter panel (Refer to sample attached). If the Customers Service  Agent Car doesn’t lined with a 6 inches high amber strip, it must Stick a PAWOA Sticker on the top section of front windscreen and the CSA must process a Customer Service Agent card issued by FAWOAM through PAWOA.

4.4.4  No matter a tow truck or a CSA car, a PAWOA Sticker must stick on the top section of front windscreen. If the Customers Service Agent Car doesn’t lined with a an amber strip, it must process a sticker on the top section of front windscreen and the CSA must process a Customer Service Agent card issued by FAWOAM through PAWOA. In another word, a CSA car must have sticker & the amber strip on both sides can be exempted. But the CSA  who is on duty must process a CSAC issued by FAWOAM through PAWOA.
4.4.5  If the applicant is not the owner of the vehicle he used for providing service ,he must get a consent letter from the vehicle owner. The consent letter must be chop & signed by the workshop. (Refer to sample attached)
4.4.6  Wear a proper shoe (cover the whole foot), not a slipper.
4.4.7  On your way to accident scene, CSAC Car cannot use beacon light. You can only use it when  you are completely stopped at the accident scene.


4.5  Any card holder who wish to change a new vehicle, PAWOA will issue a replacement if he submits a copy of his photo and a photo copy of his new vehicle registration card.
4.6  When a Customer Service Agent providing a service, he cannot use any type of Lorry,Van, MPV  or 4-wheel Drive . 

                       
Those workshops who wish to apply for 2025 Customer Service Agent Card, are requested to fill up the application forms and send together with all the above mentioned  documents and payment to PAWOA  secretariat at your earliest convenience. Please give us one week to process your application. Please note that Application Without  payment attached, will not be entertained. Thank you.
